Iranian officials admitted Saturday that their military accidentally shot down a Ukrainian jetliner, killing all 176 aboard.

The mistake was blamed “human error.”

The airliner was a Boeing 737 operated by Ukrainian International Airlines. It was shot down near the outskirts of Tehran during takeoff only hours after Iran launched missiles at U.S. forces in Iraq.

Why the airliner was allowed to take off during active missile launches is still unclear.
